Dynamic Forms

As noted, forms are a valuable way to collect information, but Dynamic Forms expand that value exponentially.

Most of today’s form designers offer auto-fill, date pickers, and simple calculations, but truly Dynamic Forms developed by an experienced designer, using quality software can catapult your data collection to higher levels. Some advanced options include;

Adobe AEM Forms – I’m well versed in using AEM Forms, however all of the forms I designed using this software were under a binding Nondisclosure agreement.

Adobe LiveCycle Designer – I’m especially proud of the Dynamic Forms I built while working for the Texas Department of Aging and Disability Services (DADs), some of which are available below. Built using Adobe LiveCycle Designer, these forms include a variety of advanced dynamic abilities.

Form H0010 – Application for Advisory Committee Membership

Form H0010 – Developed to collect information about council member applicants.

Form 4741-HHS – On-Site Event/Training Request

Form 4741 – Used by DSHS staff to request events, training, or seminars.

Form 3613 – Investigation Report and Cover

Form 3613 – Used by long-term care providers to document self-reported incident investigation summary, analysis and findings in accordance with regulatory requirements.

Form 2371 – In-Home and Family Support Program Financial Worksheet

Form 2731 – Used to verify income of an individual applying for an In-Home Family Support Grant.

The In-Home and Family Support program was a grant program that offered financial support for physically disabled individuals. The 85th Texas legislative session chose to end the program on Aug. 31, 2017.

Form 5836 – Form Letter

Form 5836 – Used by DADS staff to notify contractors of potential termination of their contract. There were several of these types of form letters created. Fields were added to increase the flexibility of the letter while maintaining uniformity.
