Simple Forms

From 2004 to 2008 I worked for Brushy Creek Municipal Utility District (BCMud). I learned to use Adobe Acrobat Pro 8 and designed forms for the district’s use. These new forms replaced older and less organized ones that were available only in photocopied paper format.

The forms I designed were in .pdf format and posted online for customers to print, complete, then deliver to the district.

My designs are still in use, some only slightly modified and set to offer data entry. You can link to BCMud here and see the forms are still in use.
